About the Role

Join the Sub Acute Ambulatory Care team as an Intake Worker and play a key role as the first point of contact for patients, carers and referring organisations. This position supports access to Community Allied Health and Sub-Acute Non-Admitted Services through referral management, initial screening and care coordination. Working within a collaborative multidisciplinary environment, you will help connect consumers with the right services while promoting a person-centred approach to care.

 

Key Responsibilities

• Manage enquiries and referrals as a single point of contact

• Assess eligibility, priority and funding pathways

• Complete intake screening and identify service needs

• Schedule initial appointments and coordinate access to care

• Maintain accurate referral records and iPM data entry

• Provide feedback and communication to referrers and stakeholders

 

Skills & Experience

• Registered Nurse, Enrolled Nurse or Allied Health Professional

• Strong communication and interpersonal skills

• Customer service experience, preferably in healthcare

• Proficient in Microsoft Office, Excel and Outlook

• Positive, organised and adaptable approach to work

  • If you are a Registered Nurse, this position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Registered Nurse Grade 2 (YP2 - YP9) which ranges from $42.60 - $54.40 per hour.

 

  • If you are an Allied Health Professional - this position is paid in accordance with the Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021 – 2026, in reflection of the relevant Grade 2 classification.
