About the Role

The Practice Registered Nurse is responsible for delivering high-quality, evidence-based nursing care and clinical services to patients attending both Alan Wolff Medical Centre and Read Street Medical Clinic. These clinics provide specialist and general practice healthcare services to patients across Horsham and surrounding communities, working in collaboration with permanent and visiting specialists and general practitioners.

As a Registered Nurse (RN), you are a valued member of the multidisciplinary healthcare team, committed to delivering safe, effective and patient-centred care. You will utilise your clinical knowledge, skills and professional judgement to achieve the best possible outcomes for patients.

Your interactions with patients, families, specialists, general practitioners and colleagues will be guided by the organisation's values of patient-centred care, trust, respect and professionalism. You will contribute to a culture of continuous quality improvement and support initiatives that enhance patient safety and the overall patient experience.

 

Key Responsibilities

  • Deliver high-quality, evidence-based nursing care and assist with clinical procedures as directed by General Practitioners and Specialists, including but not limited to pathology collection, electrocardiograms (ECGs), Holter and blood pressure monitor application, wound management, minor surgical and gynaecological procedures, immunisations, and the application and removal of plaster and synthetic casts.
  • Conduct patient assessments, pre-consultation screening and clinical observations, including vital signs and diagnostic measurements, and administer medications and injections in accordance with legislative requirements, clinical protocols and scope of practice.
  • Ensure the clinic environment is maintained to a high standard, promoting patient safety, infection prevention and efficient service delivery through effective stock management, equipment maintenance, and adherence to Cold Chain Management requirements.
  • Monitor, maintain and coordinate the servicing, calibration and repair of clinical equipment to ensure operational readiness and compliance with safety and accreditation standards.
  • Provide patient education, health promotion and preventative health advice that supports informed decision-making, self-management and improved health outcomes.
  • Deliver person-centred nursing care that upholds the rights, dignity, cultural diversity, privacy and confidentiality of all patients and their families.
  • Advocate for patients by supporting their participation in care planning and decision-making whilst prioritising patient safety, wellbeing and quality healthcare outcomes.
  • Practice in accordance with the Nursing and Midwifery Board of Australia (NMBA) Registered Nurse Standards for Practice, relevant legislation, professional codes, organisational policies and current clinical guidelines.

 

Skills & Experience

Essential

  • Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrate excellent communication and interpersonal skills, with the ability to build effective relationships with patients, colleagues, and external stakeholders.
  • Proven ability to work collaboratively and effectively within a multidisciplinary healthcare team.
  • Sound clinical assessment, decision-making, and problem-solving skills, with the ability to prioritise and respond appropriately to patient needs.
  • Well-developed written communication, documentation, and computer literacy skills, including experience using clinical software and electronic health records.
  • Demonstrated ability to work autonomously, exercise initiative, and effectively manage competing priorities in a dynamic healthcare environment.
  • Strong organisational and time management skills, with a commitment to delivering high-quality patient care.
  • Commitment to maintaining professional standards, confidentiality, and ongoing professional development.

Desirable

  • Postgraduate qualification primary health care, chronic disease management, immunisation, or a related discipline.
  • Previous experience in general practice nursing or a primary healthcare setting.
  • Accreditation in nurse immunisation and/or chronic disease management programs.
  • Experience with quality improvement activities, health assessments, care planning, and preventive health initiatives.

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Registered Nurse Grade 2 (YP2 - YP9) which ranges from $42.60 - $54.40 per hour.
